Payment Plans
South Florida Joint Replacement Center may offer payment plans to eligible patients who cannot pay their estimated financial responsibility before or on the date of service. Payment plans are subject to the Center’s Point of Service Policy and approval requirements.
Features of Our Payment Plans:
- Eligibility: The deductible must be satisfied, and the patient must have applied for CareCredit and received a denial letter.
- Exclusions: Payment plans are not offered for deductibles, copays, or self-pay procedures.
- Option 1: Pay 50% upon admission and the balance within 30 days.
- Option 2: Pay 50% upon admission and the balance within 60 days. If needed, monthly payments may continue until the balance is paid in full, not to exceed six months.
- Option 3: Pay 33% upon admission and the balance within 60 days. If these terms cannot be met, the account is referred to the CEO or Business Office Manager for review.
- Automatic Payments: An approved plan requires a credit card on file for automatic withdrawals until the balance is satisfied.
Who Can Apply: Eligible insured patients with an out-of-pocket balance other than a deductible or copay may request a payment plan. Self-pay procedures are not eligible for payment plans.
How to Get Started: Contact Yvonne Rosato, Business Office Manager, at 954-904-0723 before the date of service. She will review eligibility, payment-plan options, required approvals, and automatic-payment terms. If an approved plan defaults, the remaining balance may be referred to collections and the patient may be ineligible for future payment plans.
