Financial Assistance

South Florida Joint Replacement Center is committed to making medically necessary surgical care accessible to eligible patients experiencing financial hardship. Financial assistance is administered under the Center’s approved Charitable Care Policy and does not apply to cosmetic procedures.

Eligibility Criteria: Patients may request an individualized review under the Charitable Care Policy. Eligibility and any discount or payment arrangement are determined by the Center on a case-by-case basis using the applicable criteria and required supporting documentation.

How to Apply: Contact Yvonne Rosato, Business Office Manager, at 954-904-0723 to request a financial assistance review. She will explain the documentation required and the approval process.

• Approved charitable care or financial-hardship determinations may supplement or replace standard self-pay pricing, as authorized by Center policy.
• All approvals and supporting documentation are retained in the patient record.
